A man died after suffering severe brain injury during a warm up jousting match for Channel 4's Time Team, reports say.
Paul Allen, 54, was participating in a warm up jousting match ahead of filming when a piece of balsa wood tip flew through the eye-slit of his helmet, penetrating 5 inches through his eye and into his head causing severe brain injury.
It was said he died of severe penetrating brain injury along with cardio-respiratory failure.
Coroner Anne Pember said the accident was down to the program's failure to use someone with a proven record of lance-breaking jousting, ensure the shield was properly constructed and to make sure the appropriate helmet was utilised.
This comes after a Welsh woman died after suffering severe brain injury when she fell down the stairs in her own house.
Brenda Davies left her bedroom for the spare room to escape her husband's snoring but fell down the stairs on the way, which resulted in the fatal brain injury.
