Nottinghamshire children's and health services have been criticised for their handling of the case of a baby who suffered a brain injury that led to her death.
Services in place that could have prevented the death of child D, as she is referred to in the report, were not adequate, the report claims.
It highlighted information sharing between children's services, the NHS and the police as being below standard.
This is Nottingham reports that child D was a 35-day-old baby, who was in the care of her father when it is likely that she sustained a fall. He pleaded guilty to manslaughter.
The paper reports that he had a history of violence and mental health problems.
Falls can cause a type of accident known as traumatic brain injury (TBI), which the charity Headway suggests is chain of three linked events.
It says that the first event is contained within the first few seconds of the impact and the second when the brain becomes starved of oxygen. The third event is when serious complications arise, such as blood clots causing brain damage.
